| 非哺乳期乳腺炎(non-puerperal mastitis,NPM)是一类以慢性非特异性炎症为特征的疾病,尤其好发于中青年女性。研究表明,NPM病灶中常伴有局部的炎症微环境改变与异常脂质沉积,脂代谢紊乱所致的免疫炎性激活被认为是NPM发生发展的重要病理机制。中医理论认为,痰浊凝滞,壅塞乳络是NPM的重要病机之一。究其本源,乃脏腑内虚,气化不利,痰浊内生,阻滞乳络,三焦失调而成。本文基于痰浊理论,结合脂代谢异常与炎症调控的相关研究,分析二者在NPM发病中的内在联系,从病机层面阐释中医药在调控炎症反应与脂代谢失衡中的作用内涵,为痰浊理论的现代诠释提供新的病机视角。 |

| Non-puerperal mastitis (NPM) is a disease characterized by chronic nonspecific inflammation and occurs predominantly in young and middle-aged women. Studies have shown that NPM lesions are frequently accompanied by alterations in the local inflammatory microenvironment and abnormal lipid deposition. Immune-inflammatory activation induced by lipid metabolism disorders is considered an important pathological mechanism underlying the initiation and progression of NPM. According to traditional Chinese medicine (TCM) theory, stagnation of phlegm-turbidity obstructing the breast collaterals represents one of the key pathogenic mechanisms of NPM. Fundamentally, it arises from deficiency of the zang-fu organs, impaired qi transformation, internal generation of phlegm-turbidity, obstruction of the breast collaterals, and dysfunction of the Sanjiao. Based on the theory of phlegm-turbidity, and in conjunction with current research on lipid metabolism abnormalities and inflammatory regulation, this paper analyzes their intrinsic relationship in the pathogenesis of NPM. From the perspective of pathogenesis, it further elucidates the role of traditional Chinese medicine in regulating inflammatory responses and lipid metabolic imbalance, providing a new mechanistic perspective for the modern interpretation of the phlegm-turbidity theory. |
