      近年来溃疡性结肠炎的发病率呈逐年上升趋势,中医在治疗溃疡性结肠炎方面有一定的特色与优势。“五脏苦欲补泻”理论源于《黄帝内经》,旨在根据五脏的生理特点,通过药物的五味属性来调节五脏功能。该理论为临床组方以及药物选择提供了重要指导。本文认为,该理论对UC的指导价值在于揭示了其从活动期“肝苦急、脾苦湿”向缓解期“肾苦燥、肺气虚”演变的核心病理传变链。据此,治疗关键在于动态把握疾病阶段,运用五味实现“调中轴(脾)、疏肝木(肝)、固根本(肾)”的靶向调节。本文通过系统阐释“苦甘调脾”、“辛酸理肝”等具体治法,旨在为该经典理论在UC治疗中的现代化应用提供理论参考与临床思路。

      In recent years, the incidence of Ulcerative Colitis (UC) has shown an upward trend, and Traditional Chinese Medicine (TCM) offers unique characteristics and advantages in its treatment. The theory of "Supplementation/Drainage for Bitter-Desire of the Five Viscera," originating from Huangdi Neijing (The Yellow Emperor"s Inner Canon), aims to regulate the functions of the five viscera based on their physiological characteristics by utilizing the five flavors of medicinals. This theory provides significant guidance for clinical prescription and medicinal selection. This paper posits that the guiding value of this theory for UC lies in revealing its core pathological transmission chain, which evolves from the "Liver"s bitter-desire for urgency and Spleen"s bitter-desire for dampness" in the active stage to the "Kidney"s bitter-desire for dryness and Lung qi deficiency" in the remission stage. Accordingly, the key to treatment is to dynamically grasp the disease stage and apply the five flavors to achieve targeted regulation of "harmonizing the central axis (Spleen), soothing the Liver-wood, and consolidating the root (Kidney)." By systematically elaborating on specific therapeutic methods such as "regulating the Spleen with bitter and sweet flavors" and "managing the Liver with pungent and sour flavors," this article aims to provide theoretical references and clinical insights for the modern application of this classic theory in the treatment of UC.
