      目的 观察苦参汤加味熏蒸坐浴配合红光照射应用于混合痔吻合器痔上黏膜环切术(PPH)后的临床效果。方法 采取前瞻性对照研究法,按随机数字表法将丽水市人民医院收治的106例混合痔行PPH手术治疗患者分为观察组、对照组(各53例),其中观察组完成试验52例(剔除1例),对照组完成试验50例(剔除3例)。对照组术后给予高锰酸钾溶液坐浴配合红光照射治疗,观察组给予苦参汤加味熏洗坐浴配合红光照射治疗,比较2组肛周疼痛、创面愈合情况、血清炎症反应及术后症状缓解时间、创面愈合时间。结果 治疗14d后,观察组总有效率96.15%,优于对照组84.00%(P<0.05);治疗7d、14d后,2组肛周疼痛、创面面积、创面水肿及创面渗液评分均不同程度降低,观察组低于对照组(P<0.05);2组治疗7d后创面肉芽组织评分比较无显著性差异(P>0.05),治疗14d后创面肉芽组织评分均提高,观察组高于对照组(P<0.05);治疗7d、14d后,2组血清炎症指标[C反应蛋白(CRP)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平比较均不同程度降低,观察组低于对照组(P<0.05);观察组术后疼痛缓解时间、水肿消失时间、渗液消失时间及创面愈合时间均早于对照组(P<0.05)。结论 采用苦参汤加味熏蒸坐浴配合红光照射治疗混合痔PPH术后患者疗效显著,对缓解术后肛周疼痛、促进创面愈合、减轻炎症反应均具有积极作用,值得推广借鉴。

      Objective: To observe the clinical effect of modified Kushen decoction fumigation sitz bath combined with red light irradiation on mixed hemorrhoid stapler hemorrhoidectomy (PPH). Method: A prospective controlled study was conducted, and 106 patients with mixed hemorrhoids who underwent PPH surgery at Lishui People"s Hospital were randomly divided into an observation group and a control group (53 cases each) using a random number table method. Among them, 52 patients in the observation group completed the experiment (excluding 1 case), and 50 patients in the control group completed the experiment (excluding 3 cases). The control group was treated with potassium permanganate solution sitz bath combined with red light irradiation after surgery, while the observation group was treated with modified Kushen decoction fumigation sitz bath combined with red light irradiation. The perianal pain, wound healing, serum inflammatory response, postoperative symptom relief time, and wound healing time were compared between the two groups. After 14 days of treatment, the total effective rate of the observation group was 96.15%, which was better than the control group"s 84.00% (P<0.05); After 7 and 14 days of treatment, the perianal pain, wound area, wound edema, and wound exudate scores in both groups decreased to varying degrees, with the observation group being lower than the control group (P<0.05); After 7 days of treatment, there was no significant difference in the granulation tissue scores between the two groups (P>0.05). However, after 14 days of treatment, the granulation tissue scores in the observation group increased significantly compared to the control group (P<0.05); After 7 and 14 days of treatment, the levels of serum inflammatory markers [C-reactive protein (CRP), interleukin-1 β (IL-1 β), and tumor necrosis factor - α (TNF - α)] in both groups decreased to varying degrees, with the observation group being lower than the control group (P<0.05); The observation group had earlier postoperative pain relief time, edema disappearance time, exudate disappearance time, and wound healing time than the control group (P<0.05). Conclusion: The use of modified Kushen decoction fumigation sitz bath combined with red light irradiation has significant therapeutic effects on patients with mixed hemorrhoids after PPH surgery. It has a positive effect on relieving postoperative perianal pain, promoting wound healing, and reducing inflammatory reactions, and is worthy of promotion and reference.
